We support individuals with intellectual and developmental disabilities (I/DD) through Medicaid-funded services. Our person-centered planning process ensures that each individual’s needs are met, helping them develop daily living skills, personal care, social skills, and community involvement. We also offer professional nursing care to provide respite for caregivers.

Residential Habilitation (ResHab)
What is ResHab?
Our Residential Habilitation Services, commonly known as ResHab, provide comprehensive assistance and training. These services extend to activities of daily living, independent living skills, typical household activities, family functions, community events, and social and leisure skill development.
We understand the unique challenges that individuals with I/DD face. Our dedicated team is committed to helping them develop and maintain daily living skills in a nurturing residential environment, focused on personal care, social skills, and community integration.

Personal Assistance Habilitation (PAB)
What is PAB?
Personal Assistance/Habilitation (PAB) consists of assistance or training to enable participants to acquire, retain, or improve skills related to living in the family home or their own home — activities of daily living such as bathing or dressing, independent living skills such as laundry or cooking, and communication and interpersonal skills.
Our one-on-one support for individuals with I/DD is far from one-size-fits-all. We tailor our assistance to each person’s unique needs, helping them navigate daily activities while fostering independence, confidence, and personal growth.

Respite & Respite RN/LPN
What is Respite?
Respite and Respite RN/LPN services offer short-term care for participants residing in family homes, relieving those who typically provide unpaid care. If nursing assessment, judgment, and skilled interventions are required, the service may be delivered by an RN or LPN under the supervision of an RN.
Caregiving can be both rewarding and demanding. Our professional nursing care offers temporary relief for primary caregivers, ensuring quality care for their loved ones while they rest and recharge — providing relief, peace of mind, and a sense of being supported.

Community Learning Services (CLS)
What is CLS?
Community Learning Services (CLS) are provided in the community to support participants in improving their access to the community by enhancing their skills, improving communication, developing and maintaining friendships, and gaining experience with available opportunities.
We acknowledge the significance of community involvement and belonging. Our personalized program supports individuals with I/DD in engaging in community activities and developing essential social and vocational skills, encouraging growth and meaningful connections.

Chore Services
What is Chore?
Chore services are essential for maintaining a clean, sanitary, safe home environment. These services are available to participants living in their homes who are without natural (non-paid) support or with family members who cannot perform the tasks.
We acknowledge the significance of community involvement and belonging. Our personalized program supports individuals with I/DD in engaging in community activities and developing essential social and vocational skills, encouraging growth and meaningful connections.
